Mobile Clinic School Health
The CPS Office of Student Health & Wellness (OSHW) Primary Care Services team is pleased to share a series of Saturday School Health Clinics being offered this fall in partnership with the Chicago Department of Public Health (CDPH), PRISM Health, and City Colleges of Chicago.
These events are open to families with babies, toddlers, children, teens, and young adults through age 21 and are intended to make required and preventive health services more accessible to families across Chicago.
All clinics will take place on Saturdays from 9:00 a.m.–1:00 p.m.

Services may include:
- School physical exams
- Sports physical exams
- School-required vaccinations
- Blood lead risk screening and testing
- Vision and hearing screenings
- Dental exam referrals
- Social-emotional check-ins
- Growth and nutrition checks
- Benefits navigation assistance
Services may vary by event and are available while supplies last.
Families are encouraged to bring their child’s available health and immunization records. Families with insurance should also bring their insurance card. Families without insurance may qualify for reduced-cost services based on their ability to pay, and no family will be turned away solely because they cannot pay.
Families can register in advance by calling 800-325-1812. Walk-ins will also be accepted while supplies last.
