Dear Families,
As a reminder: All Kindergarten, 2nd grade, 6th grade, and 9th grade must have a dental exam completed by a licenseddentist before May 15th of the school year. In partnership with the Chicago Department of Public Health (CDPH), CPSwill offer on-site, preventative dental services at no cost to ALL Pre-K-12th grade students. Schools are assigned adentist and will see all students with a signed consent form.
The dental teams can provide students with:
● Dental Exams
● Dental Cleanings
● Fluoride treatment
● Dental sealants, as needed
● and referrals, as needed
What to do next?
1. Please determine who the dental program liaison will be at the school to help coordinate with our dental
providers. Outside of the required grades we are also prioritizing STLS students. The program is intended
for any student in need of dental care.
2. When scheduling dental services with the dentist please CC’ me on any communication.
3. A minimum of 50 signed and completed consent forms are required in order for the provider to service the
school.
4. Assign appropriate space (classroom, library, auditorium) – clutter-free to allow for spacing for students.
Every effort will be made to avoid disruptions to your instructional program, as an average dental exam is
only 15 minutes per student.
5. Assist with managing students the day of service (Classrooms/students will not commingle during dental
services.)
6. If you do not see your school assignment, please email me and we can work to coordinate that service or
provide another opportunity.

9. Dual-Billing Letter–click here: English / Spanish. We are also attaching a letter to share with parents for
those who have a Medicaid dental provider and choose to participate in our program. In the past, we have
heard that providers have denied services due to participating in our dental program. However, by state law
there is a policy called Dual-billing that allows for both our school based dental program provider and the
student’s provider (if medicaid) to bill families for dental services.

● We are also providing Oral Health Education for all CPS students with our partner, The Oral Health Forum!
